Liability Release Form - Personal Mobility Devices at Camp Eagle

This release form addresses Personal Mobility Devices {including: scooters, ATVs (All-Terrain Vehicles), LSVs (Low Speed Vehicles) or UTVs (Utility Terrain Vehicles) or similar vehicles} at Camp Eagle. A mobility device to assist in navigating the Camp Eagle Campus is permitted. You must bring your own personal mobility device. The following release must be completed before allowing your personal mobility device on the Camp Eagle Campus.

You agree to be the sole driver of your mobility device. No other persons will be permitted on the device or drive your device under any circumstances. You agree to keep it secure when not in operation.

Camp Eagle makes no warranties, expressed, implied or apparent regarding your vehicle.

As the sole operator of your equipment, Camp Eagle will not be held responsible for any damage to, loss or theft of your mobility device including damage caused by “Acts of God” or terrain conditions. You are responsible for any and all damages to anything or anyone caused by this device.

You agree to indemnify Camp Eagle, defend Camp Eagle and hold Camp Eagle harmless from all claims, liabilities, costs and attorney fees, due to the use of your mobility device while at Camp Eagle.

You will drive respectfully and have the upmost interest for others on campus.

Camp Eagle will not be responsible for vehicular consumables.

You will not exceed the posted speed limit while on the Camp Eagle campus.

The authorized driver will be 18 years old or older and submit a copy of your driver’s license.
